We welcome new patients, and we’re committed to making sure that you have a great experience with us. When you contact us, please let us know if you have any questions or concerns, or if we can better accommodate you in any way. We are also happy to schedule your first appointment in conjunction with an oral hygiene appointment to make it more convenient for you.

In order to ensure that we have all of the information we need about you to provide you with the best dental care possible, we require all new patients to complete our New Patient Form.
If you would like to complete the form prior to arriving at our office, please download and complete the form. This form can be filled out electronically in Acrobat. Once you've completed the form simply e-mail the completed form
to smiles@riverdentistry.ca.
Otherwise, we will have you complete the form upon your arrival at our office.
